1. Introduction

Welcome to Orkida Dakoli PMU / Formby Microblading Phibrows. By booking an appointment and using our services, you agree to comply with and be bound by the following terms and conditions. Please review these terms carefully before proceeding with your appointment.

2. Services Offered

Orkida Dakoli PMU offers permanent makeup services, including but not limited to:

Microblading

Ombre Powder Brows

Nano Brows

Lip Blush

Other cosmetic tattooing services

The specifics of each treatment, including technique, longevity, and aftercare, will be discussed during the initial consultation.

3. Booking and Appointments

3.1 Deposit Requirements

A non-refundable deposit of £50 is required to secure your appointment. The deposit will be deducted from the total service cost on the day of your appointment.

Appointments are not confirmed until the deposit is paid.

3.2 Cancellation Policy

Clients must provide at least 72 hours' notice for cancellations or rescheduling.

Failure to provide sufficient notice may result in the forfeiture of the deposit.

3.3 No-Show Policy

If a client fails to attend the appointment without prior notice, the deposit will be forfeited, and a new deposit will be required to rebook.

4. Client Responsibilities

4.1 Consultation and Medical Disclosure

Clients must disclose any relevant medical conditions, allergies, or medications during the initial consultation.

We reserve the right to refuse treatment if it is deemed unsafe based on the medical information provided.

4.2 Age Requirements

Clients must be at least 18 years old to receive permanent makeup services. Valid identification may be required.

4.3 Pre-Treatment Instructions

Clients must follow all pre-treatment instructions provided to ensure optimal results and avoid complications.

5. Aftercare and Results

5.1 Aftercare Instructions

Detailed aftercare instructions will be provided after the treatment. Clients must adhere to these instructions to achieve the best results.

5.2 Touch-Ups and Maintenance

The initial cost includes one complimentary touch-up session within [X] weeks after the first procedure. Any additional touch-ups will incur an extra charge.

5.3 Longevity and Expectations

The results of permanent makeup may vary depending on skin type, lifestyle, and aftercare adherence. Orkida Dakoli PMU cannot guarantee specific outcomes.

6. Refund Policy

Due to the nature of the services provided, all payments are final. No refunds will be issued for completed services.

Deposits are non-refundable, except in the case where Orkida Dakoli PMU cancels the appointment without rescheduling options.

7. Liability Disclaimer

Orkida Dakoli PMU is not responsible for any adverse reactions that occur as a result of the treatment or failure to follow aftercare instructions.

Clients agree to release and hold harmless Orkida Dakoli PMU, its employees, and contractors from any and all claims or liability arising out of or in connection with the services provided.

8. Privacy Policy

Orkida Dakoli PMU respects your privacy. Any personal information collected during the booking process is used solely for appointment management and will not be shared with third parties.

9. Changes to Terms

We reserve the right to amend these terms and conditions at any time. Any changes will be posted on our website and will become effective immediately.

10. Governing Law

These terms and conditions are governed by the laws of England and Wales. Any disputes arising from these terms shall be subject to the exclusive jurisdiction of the courts of England.
